Application forms and instructions for their submission can be obtained from:

The completed application should be returned to the above address, together with the application fee of $300.00, none of which is refundable. Only persons who believe they clearly meet the stated qualifications and requirements for Certification should submit applications.
A recent photograph must accompany the application. The applicant must
also identify three professional and character references from persons (other
than Directors of the Board) knowledgeable about the applicant’s forensic
toxicology experience, to be sent directly to the Board’s office by each writer. Official transcripts from each college or university attended (irrespective of degrees received) also must be sent directly to the Board's office by the registrars.
Completed applications are reviewed by the Credentials Committee of the Board, who vote on whether or not to admit the applicant to the examination.
Examinations for accepted candidates are prepared and evaluated by the Examination Committee, whose recommendations are considered, as expeditiously as possible, by the full Board of Directors for final action.
Successful candidates are issued a Certificate of Qualification in Forensic Toxicology by the Board to attest to their status as Diplomates of the American Board of Forensic Toxicology, and are listed in the next revision of the directory of Diplomates.
Qualifications, requirements, and application procedures for certification are subject to revision by the Board. The latest version is always obtainable from the above address.
